Welcome to the RMHS download forms page. If your son or daughter is going to any of our standard RMHS development camps all you have to do is fill print and fill out the RMHS Health and Registration form and bring this with you to check-in on day one. Rest assured that if you forget our staff will have some forms for you to fill out when you get to the rink on day one during check-in. (no camper without this form will be allowed in any on or off-ice programs until the RMHS staff has this on hand)

If you are going to the Rocky Mountain Adventure Camp you must fill out the following forms: If you are only participating in the hockey part of the program all you have to fill out is the health and waiver form. If you are participating in both the hockey and adventure program you must print and fill out all the forms below the Rocky Mountain Adventure Hockey Camp section.

All of these forms must come with you on day one at check in as well.
